We're excited to invite you to our annual in-person meeting. Together, we'll plan for the upcoming National Health Care Decisions Day (NHCDD) press conference and Advance Care Planning (ACP) workshops.

This meeting is a unique opportunity to:

• Share your ideas and vision for the event's direction.
• Exchange insights about successful ACP workshops and strategies.
• Connect face-to-face with others who share a commitment to improving access to high-quality serious illness care.
• Meet our new ACP Internal Champions from organizations supporting the Latinx community, who will be sponsoring NHCDD events.
