You play a vital role in deciding what's most important in your medical care. This is true for all your health care decisions, but it's especially important if you find yourself unable to communicate your preferences.
Join fellow San Franciscans on April 16th in honor of National Healthcare Decisions Day to learn about advance care planning and steps you can take to take control of your health and health care decisions.
Advance Care Planning: A Path to Empowerment
Advance care planning is a vital process designed to prepare people for effective communication and decision-making about their medical care. This proactive approach enables patients, along with their family and friends, to ensure their medical preferences are known and respected, thereby alleviating family stress and increasing the likelihood that they receive care consistent with their wishes.
Engage in the Conversation
Eighteen organizations have united to highlight the importance of advance care planning. Together, we are hosting a press conference and a series of workshops throughout San Francisco. Our goal is to shine a light on the crucial role of advance care planning and to support individuals in preparing for medical decisions. This includes contemplating what matters most to you, selecting someone to make decisions on your behalf if you are unable to do so, and learning how to communicate your future health care preferences with your family, friends, and health care providers. These workshops, offered in multiple languages, will promote culturally sensitive discussions about advance care planning. They are available via Zoom or in person, hosted in trusted community settings throughout the city. Learn more about advance care planning here.
