You play a vital role in deciding what's most important in your medical care. This is true for all your health care decisions, but it's especially important if you find yourself unable to communicate your preferences.
Starting April 16, in honor of National Health Care Decisions Day, join fellow San Franciscans to learn about advance care planning and the steps you can take to take control of your health and medical decisions.
Advance Care Planning: A Path to Empowerment
Advance care planning ensures that your medical wishes are known, respected, and honored. It fosters meaningful conversations between you, your loved ones, and your health care providers—reducing stress for your family should they ever need to make health care decisions on your behalf.

Join us on April 16 for a Kickoff Community Event and Press Conference at On Lok 30th Street Senior Center to mark National Health Care Decisions Day and help raise awareness about the importance of planning for the future.

Join a Workshop
We are also hosting a series of free workshops throughout April in partnership with community leaders across San Francisco. These workshops will help individuals prepare for medical decision-making and will be available in English, Spanish, and Cantonese to ensure accessibility for all.
